Job summary

Frontpoint Health is seeking a Physical Therapy Assistant (PTA) to provide in-home physical therapy services in a PRN/Per Visit capacity. You will support the supervising physical therapist, implement treatment plans, and monitor patient progress.

The role requires PTA certification and home health experience, strong communication, reliable transportation, and the ability to work independently within a team. Kinnser/WellSky experience is a plus.

Qualifications

• Must hold PTA certification/licensure.
• Experience providing PT services in home health.
• Ability to work independently and in a team.
• Excellent communication and interpersonal skills.
• Strong organization and time management skills.
• Valid drivers license and reliable transportation.
• Kinnser/WellSky experience preferred.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Assist the physical therapist in developing and implementing treatment plans.
• Monitor and document patient progress and adjust treatment plans as needed.
• Communicate with physicians, nurses, and other healthcare professionals regarding patient care.
• Provide education to patients and their families regarding home exercise programs and other aspects of their care.
• Ensure compliance with all regulatory and accreditation standards.
• Maintain accurate and complete patient records.
• Participate in team meetings and case conferences.
• Other duties as assigned.
