Qualifications

• Current Nursing Council registration and Annual Practising Certificate.
• At least five years of clinical experience in cardiac and/or respiratory nursing.
• Postgraduate nursing qualifications or equivalent experience.
• Proven ability to work in multidisciplinary teams and build relationships.
• Excellent assessment, communication, education and problem solving skills.

Responsabilités

• Provide specialist nursing expertise and clinical leadership for long term respiratory and cardiac conditions.
• Manage referrals for Community Oxygen Therapy, Pulmonary Rehabilitation and Heart Failure Rehabilitation.
• Deliver assessment, education and rehabilitation in community settings, gyms, virtual platforms and one-to-one appointments.
