Job summary

HarmonyCares delivers in-home primary care for high-acuity geriatric patients. This Advance Practice Provider role focuses on comprehensive, patient-centered care with minimal emphasis on volume, Monday–Friday, no weekends or evenings.

Expect use of EMR and clinical scribe tools to support charting during the workday. We seek clinicians who are dedicated to quality care, enjoy collaborative teamwork, and value CME/licensing support including annual dues.

Qualifications

• Licensed NP/PA with state license.
• DEA license/eligibility required.
• Geriatrics experience preferred.
• CPR/BLS certification active.
• Board certification preferred.
• Valid driver's license with clean record.

Overview

Value-based Advance Practice Provider managing chronic conditions of high-risk geriatric panel Monday-Friday, no weekends or evenings. Use Athena EMR + Marvix AI Scribe to get charting done during the workday. Start day around 8AM - end around 4:30/5PM.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record
