Overview

Join to apply for the Zorgcoördinator (Verzorgende / Verpleegkundige) role at Stichting Land van Horne.

Responsibilities

• Operate as a Zorgcoördinator while also performing daily care as Verzorgende or Verpleegkundige; you are the hub of the care process and work with your team to deliver care aligned with residents’ wishes.
• Ensure the right care, coordinated with the team, is matched to the residents’ needs and preferences, and is reflected in the care plan.
• On the ground on location, you contribute at a high level through additional responsibilities and tasks within your role; you monitor and coordinate care based on the client’s care plan and agreements.
• Discuss and coordinate with colleagues to ensure proper care and act as a sounding board for clients and their mantal caregivers.
• Engage with clients and their first contact person to set direction and communicate this to the team; maintain contact with disciplines such as ergotherapists, logopedists, chaplain, and other services (e.g., hairdresser, pedicure).
• Coach and guide team members to properly align high-quality care, and maintain good coordination to achieve client and colleague satisfaction.
• Involve and support the care planning process, including intake conversations, care plan discussions, core team meetings, and evaluation conversations after significant events.
• Act as the primary contact for clients and their family or representatives and regularly engage with them to coordinate care.
• Location: Mariënburght in Budel, contributing to the delivery of high-quality care for residents.

What you will do in practice

• Your core task: monitor the care process together with your team and address questions such as:
• Does the client receive what they want and need?
• Is the care aligned with the appropriate care intensity package, and should it be adjusted?
• Are colleagues following the agreed-upon care plan?
• Take the lead with the client and the primary contact to determine direction and communicate it to your team.
• Also perform your Verzorgende or Verpleegkundige tasks as part of your role.
• Be the go-to contact for the client and their caregiver and conduct regular conversations from intake to care plan discussions and core team meetings, including evaluations after bereavement.
• Coordinate with necessary disciplines (ergotherapists, speech therapists, chaplains, etc.) and arrange ancillary services (hairdresser, pedicure, etc.).
• Foster strong team communication and demonstrate that proper care coordination leads to high-quality care and patient satisfaction.

What we offer

• Permanent employment contract. Salary according to FWG 40 (or FWG 45 for nurses) within FWG 3.0 system, based on a 36-hour week (approx. €2,881.14 – €3,733.75 for FWG40; €3,050.60 – €4,102.26 for FWG45).
• Terms of employment in line with the CAO Verpleeg-, Verzorgingshuizen en Thuiszorg; includes 8% holiday allowance and an 8.33% year-end allowance, weekend allowance, and Goodhabitz e-learning.
