Job summary

Community Care Physicians in Niskayuna is seeking a full-time RN to join the Internal Medicine team. The office operates Monday to Friday, 8:00 AM–4:30 PM, with a competitive hourly rate and a recruitment bonus.

The role emphasizes patient documentation, vital signs assessment, triage, care coordination, and transitions of care, in a professional business-casual environment with comprehensive benefits.

Qualifications

• Active NYS RN license is required.
• Previous RN experience preferred.
• Strong computer skills and ability to document accurately.
• Knowledge of nursing principles and patient charting.

Responsibilities

• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Assess vital signs and the reason for each patient visit.
• Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.
• Support care management activities including coordination of patient needs, follow‑up and connection to resources.
• Assist with transitions of care - coordinating post‑discharge follow‑up, medication reconciliation and communication with patients and care teams.
• Prepare patient for visit as appropriate, based on office/provider requirements.

Our Community Care Internal Medicine Niskayuna office is looking for a full-time RN to join their team! This is a full-time position. The hours are: Monday-Friday, 8:00AM-4:30PM.

• This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities

• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Assess vital signs and the reason for each patient visit.
• Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.

Compensation: $32.00 - $45.50 per hour
