Job description

Overview

Assist in coordinating geriatric patient care activities to ensure effective communication, accurate documentation, timely follow-up and consistent patient education throughout the care continuum.

Support coordination of patient care across outpatient clinics, inpatient units, rehabilitation services, discharge planning and community or home care services (including Sunway Home Healthcare and Sunway Sanctuary).

Provide education to patients and caregivers on treatment plans, rehabilitation goals, discharge instructions and home care requirements in accordance with established care plans.

Support the implementation and adherence to standardized geriatric clinical pathways, including length of stay management, discharge readiness, home equipment planning and caregiver training.

Reinforce post-discharge follow-up plans and assist in ensuring continuity of care through proper coordination and communication.

Perform nursing care in accordance with hospital Standard Operating Procedures (SOPs), policies and evidence-based clinical practices.
