Job summary

KPJ Seremban Specialist Hospital is seeking a Hospital Records & Radiology Support Clerk to ensure accurate patient registration, data entry and seamless radiology workflow. You will support radiologists, technologists and billing staff, maintaining records and assisting with equipment and safety standards.

The role requires attention to detail, good communication, and the ability to work in a team or independently.

Qualifications

• Minimum: SPM or Diploma.
• Attention to detail; accuracy crucial when managing patient records and entering data.
• Basic knowledge of PACS and radiology procedures; ability to communicate with staff about imaging.
• Excellent communication and interpersonal skills; able to work in a team or independently when needed.

Responsibilities

• Patient Registration: collect and verify patient information, demographics, insurance and medical history.
• Processing Orders: ensure radiology orders are entered into the system and coordinate with staff for timely care.
• Data Entry: maintain accurate patient records including imaging results and radiology reports; verify insurance details and assist with billing inquiries.
• Medical Record Management: document and file radiology images and reports, digitally or on paper.
• Support Radiologists and Technologists: ensure equipment, records and materials are available for procedures.
• Customer Service: address patient inquiries, concerns and complaints professionally.
• Maintain Inventory: monitor radiology supplies and notify management when items run low.
• Record Keeping: obtain and record relevant patient history and imaging results accurately.
• Collaborate with PACS administrator to ensure studies are stored and sent correctly, and work with radiologists during ultrasound.
• Equipment Care: ensure ultrasound equipment is functioning; report errors to supervisory staff.
• Adhere to Standards: comply with hospital policies and safety requirements.

Job Description

• Patient Registration: Collect and verify patient information, including demographics, insurance details, and medical history.


• Processing Orders: Ensure that radiology orders are accurately entered into the system and communicate with the appropriate staff to ensure timely patient care.


• Data Entry: Maintain accurate patient records, including medical imaging results, patient files, and radiology reports To ensure all payments being made by patient Verify insurance information, assist with billing inquiries, and ensure proper coding for procedures.


• Medical Record Management: Ensure proper documentation and filing of radiology images and reports, either digitally or in paper format.


• Support Radiologists and Technologists: Assist radiologists and radiologic technologists by ensuring the necessary equipment, records, and support materials are available for procedures.


• Customer Service: Address patient inquiries, concerns, and complaints in a professional and efficient manner.


• Maintain Inventory: Monitor supplies for the radiology department and notify management when additional items are needed.


• Patient History and Records: Obtain and record relevant patient history, ensuring accurate documentation of imaging results and any other required information.


• To be competence of using all Hospital software (HITS, KCIS) Work closely with Picture Archiving and Communications systems (PACS) administrator to ensure studies are sent and stored in the PACS correctly without errors.


• Collaboration: Work closely with radiologists while they do ultrasound.


• Maintenance and Care of Equipment: Ensure ultrasound equipment is in proper working condition. Any error or malfunction liaison with the HOS.


• Adherence to Standards: Comply with hospital policies, professional standards, and regulatory requirements concerning patient care and safety.


• Maintaining and updating manuals, policies and procedures of the KPJ Seremban Specialist Hospital including risk management, ISO 9001, ISO 14001, ISO 45001, Patient Safety Goals and other international and national accreditation.


• Carry out duties as an employee as stipulated in Occupational Safety and Health Act (Amendments) 2022 and Other Regulation:- To take reasonable care at work for the safety of yourself and other persons To co-operate with your employer or any other person in the discharge of any duty To wear or use any protective equipment or clothing provided by the employer To comply any instruction or measure on occupational safety and health Ensure adhere to Standard People Practice (SPP). Ensure the policies on patients and family’s right are addressed at all time. Participate actively in any new service that is introduced. Ensure cleanliness, tidiness and maintain proper housekeeping in the services.

Job Requirement

• Education: Knowledge and Experiences: Minimum: SPM or Diploma.


• Skills & Competencies: Special skills required: Attention to Detail Accuracy is crucial when managing patient records and entering data.


• Computer and Software Proficiency Capable on Picture Archiving and Communication Systems [PACS]) Basic Knowledge of Radiology Procedures Understanding different imaging procedures (e.g., X-rays, CT scans, MRIs) can help in effectively communicating with patients and medical staff.


• Personal attributes: Excellent communication and interpersonal skills. Ability to work well in a team environment and independently when needed. Ability to handle sensitive and confidential patient information.
