Job Summary
We are seeking a qualified and detail-oriented Hospital Pharmacist to join our healthcare team. The Hospital Pharmacist will be responsible for ensuring the safe and effective dispensing of medications, reviewing prescriptions for accuracy, providing medication counseling, and collaborating with physicians, nurses, and other healthcare professionals to optimize patient care. The ideal candidate should possess strong clinical knowledge, excellent communication skills, and a commitment to patient safety and regulatory compliance.
Key Responsibilities
- Review and verify medication orders and prescriptions for accuracy, appropriateness, and potential drug interactions.
- Dispense medications accurately in accordance with physician prescriptions and hospital policies.
- Monitor patient medication therapies and recommend adjustments when necessary.
- Provide drug information and pharmaceutical guidance to physicians, nurses, and other healthcare professionals.
- Counsel patients and caregivers regarding medication usage, dosage, side effects, and storage requirements.
- Ensure compliance with hospital protocols, pharmacy regulations, and healthcare standards.
- Maintain accurate medication records and documentation.
- Manage inventory levels, medication procurement, storage, and stock rotation.
- Monitor controlled substances and ensure adherence to legal and regulatory requirements.
- Participate in medication safety initiatives, quality improvement programs, and clinical audits.
- Collaborate with multidisciplinary healthcare teams to improve patient outcomes.
- Stay updated with advancements in pharmacology, drug therapies, and healthcare regulations.
Qualifications & Requirements
- Bachelor's Degree or Doctor of Pharmacy (Pharm.D.) from a recognized institution.
- Valid Pharmacist License/Registration as per applicable regulatory authority.
- Strong knowledge of pharmacology, medication management, and clinical pharmacy practices.
- Familiarity with hospital information systems and pharmacy management software.
- Excellent analytical, organizational, and problem-solving skills.
- Strong interpersonal and communication abilities.
- Ability to work effectively in a fast-paced healthcare environment.
