Responsibilities

• Updating and ensuring the accuracy of patient health records by adding new information about diagnoses, procedures, medications, and other relevant data by listening the recorded patient calls.
• Clinical Screening – Conduct clinical screenings to determine CCM eligibility.
• Utilize the EHR to document screening findings and eligibility status.
• Collaborate with care coordination teams to transition eligible patients into the CCM program.
• Stay updated on medical terminology, procedures, and treatment protocols to ensure accuracy and consistency in transcriptions.
• Reviewing medical records to ensure they are complete and accurate.
• Diagnosing medical conditions based on symptoms presented in the medical record.
• Performing administrative tasks such as typing reports and maintaining databases.

Skills And Experience

• Post graduate (Nursing) mandatory.
• 5‑10 years overall experience (Registered nurse experience: 5 years).
• 2 years clinical experience (Bedside).
• Experience with US Health care systems is a mandatory requirement.
• Experience in Hospital Management System or Hospital Information System.
• Commitment to maintaining confidentiality and adhering to ethical standards in handling sensitive patient information.
• Ensure the integrity and confidentiality of patient information by adhering to HIPAA regulations and privacy policies.
• Excellent English professional writing skills; including advanced proficiency in grammar and spelling.
• Excellent communication & listening skills.
• Ability to understand diverse accents and dialects of physicians, their staff and patients.
• Familiarity with medical terminologies & medical summarization.
• Ability to work independently in a secure and private location with a reliable high‑speed internet connection.
• Ability to participate in live video chat and screen sharing sessions for training.
• Experience with the Microsoft Office 365 or other cloud‑based productivity tools.

Work Schedule & Location

• This role is completely remote.
• Shift timings 5:30 pm – 2:00 am (US Shift) / 3 pm – 12 am.
• Ability to work on weekends if required (comp‑off on weekdays).
