Social Worker – Adult Primary Care

The Social Worker will provide a professional social work service to adults in the Laois Primary Care area, supporting individuals, families, and carers to achieve improved health, wellbeing, and social outcomes. The post holder will work as part of a multidisciplinary Primary Care Team, addressing the psychosocial factors that impact health and functioning, and supporting clients to live independently within their communities.

Responsibilities of Social Worker – Primary Care
  • Provide a comprehensive social work service to adults presenting with a range of complex health, social, and emotional needs within the Primary Care setting.
  • Undertake psychosocial assessments and provide intervention, advocacy, counselling, and support in line with the person’s goals and needs.
  • Support clients with issues such as adjustment to illness or disability, carer stress, isolation, bereavement, and safeguarding concerns.
  • Promote early intervention and prevention to enhance wellbeing and reduce risk.
  • Liaise with GPs, public health nurses, occupational therapists, physiotherapists, and other multidisciplinary team members to provide coordinated, holistic care.
  • Work collaboratively with voluntary and statutory agencies to ensure clients have access to appropriate community resources and supports.
  • Participate in adult safeguarding processes in line with HSE policy and procedures.
  • Maintain accurate and confidential records in accordance with HSE and professional standards.
  • Maintain professional registration and engage in continuous professional development (CPD).
  • Participate in supervision, team meetings, and training as required.
  • Contribute to the education and support of social work students as appropriate.
  • Contribute to the planning, development, and evaluation of Primary Care social work services in Laois.
  • Participate in quality improvement, audit, and service development initiatives.
  • Ensure compliance with HSE policies, procedures, and governance frameworks, including health and safety and data protection.
Responsibilities of Social Worker – Adult Primary Care
  • Registered, or eligible for registration, as a Social Worker with CORU.
  • Experience working with adults in community, healthcare, or social care settings is desired.
  • Demonstrated ability to manage a varied caseload and work independently as well as part of a team.
