Job summary

Health Service Executive in Ireland seeks a dedicated professional to provide harm reduction, care planning and robust case management for individuals in the Social Inclusion cohort. You will engage with clients, coordinate interventions and work within a multidisciplinary team to ensure continuity of care.

Experience in drug/alcohol treatment settings is essential, with a track record of providing client-centred, evidence-based support and implementation of care plans across multiple counties
