Responsibilities

• Facilitate decision making regarding referrals to the MCRN.
• Assessment of rehabilitation need.
• Introduction of standardised assessment of need.
• Coordinate onward referral and seamless transfers between post-acute rehabilitation services.
• Responsible for ensuring the completion of relevant rehabilitation documentation.
• Provide clinical leadership to develop and support all staff involved in referring to post-acute rehabilitation.
