Job summary

Kerry Parents and Friends Association seeks a Community Access Facilitator in Killarney Day Services 1. You will support adults with disabilities to achieve personal goals, build confidence and participate actively in their communities.

Reporting to the Centre Manager, you will work within a Human Rights Based Approach, focusing on outcomes and inclusion, while maintaining records and collaborating with families and professionals.

Qualifications

• Qualification in General Health and Social Care or Community/Youth Work at NFQ Level 5 or higher.
• Driver licence required. Willingness to drive Association vehicles.
• Flexibility around working hours as service needs arise.

Responsibilities

• Work with each person supported to develop meaningful, person-centred goals and outcomes.
• Support adults with disabilities to exercise choices and participate in community life.
• Promote active citizenship and valued social roles for participants.
• Identify opportunities in education, volunteering, work experience and civic life.
• Support individuals in practical skills for daily living and use of community facilities.
• Provide respectful personal support including assistance with personal care and medications as required.
• Maintain high quality records and contribute to quality improvement.

Job description

Post: Community Access Facilitator

Contract: Permanent, Full Time (39 hours per week)

Location: Killarney Day Services 1

Roster: Mon, Thurs, Fri: 08:30 - 17:00 / Tues, Wed: 09:00-17:00

The Role

Reporting to the Centre Manager, or to a designated senior staff member where appropriate, the successful candidate will play a key role in supporting adults with disabilities to achieve their personal goals, build confidence and independence, and take an active part in community life. Working within the framework of New Directions, the role will focus on person centred outcomes, valued social roles, active citizenship and meaningful community inclusion, underpinned by a Human Rights Based Approach.

Key Responsibilities

• Work in partnership with each person supported and their nominated circle of support to develop and progress meaningful, person centred goals and outcomes.
• Support adults with disabilities to make their own choices, build confidence and independence, and have their voice at the centre of planning and decision making.
• Promote active citizenship and community inclusion by supporting people to develop valued social roles, friendships, relationships and natural community connections.
• Identify and support opportunities in education, training, volunteering, work experience, supported employment, social participation and civic life.
• Support individuals to develop practical skills for everyday and self directed living, including communication, travel, money management and use of community facilities.
• Provide respectful and dignified personal support, including assistance with personal care, medication and prescribed therapeutic interventions where required.
• Promote positive risk taking, and wellbeing, supporting individuals to make informed choices while identifying, managing and monitoring risk appropriately.
• Work collaboratively with families, nominated circles of support, multidisciplinary teams, employers, educational providers, advocacy groups and community organisations to achieve individual outcomes and overcome barriers to inclusion.
• Maintain high quality records and documentation, monitor individual outcomes and rights related indicators, and contribute to quality assurance and continuous service improvement.
• Promote equality, inclusion and the rights of people supported in line with New Directions, a Human Rights Based Approach and the United Nations Convention on the Rights of Persons with Disabilities.

Essential Criteria

• Possess a Qualification in General Health and Social Care or Community and/or Youth Work, as outlined in the Quality and Qualifications Ireland (QQI) formerly FETAC on the National Framework of Qualifications (NFQ) of not less than QQI Level 5. (See Appendix 1 of the Job Description for the full list of qualifications).

Or

• Possess a relevant Health Care qualification at not less than QQI level 5 on the National Framework of Qualifications (NFQ)

Or

• An equivalent Health Care qualification from another jurisdiction.
• Relevant experience working in community, social, or work-based support settings, or in any other setting that aligns with the responsibilities of this role.
• Flexibility around working hours may be required in response to service needs.
• Applicants must have the necessary authorisation to work in Ireland and will be asked to provide supporting documentation during the recruitment process.
• Hold a full driver's licence, have proven driving experience, and be willing to drive Association vehicles. Regarding use of own vehicle for work purposes, applicants must ensure it is roadworthy and covered by valid business insurance (or be willing to obtain business use insurance).

Desirable Criteria

• A minimum of one year’s experience supporting adults with an intellectual disability post qualification is preferable.
