Are you a Mental Health nurse looking for a newrole? Livewell Southwest, one of the most exciting organisations in Health& Social Care, is seeking enthusiastic practitioners to join our multi-disciplinarySpecialist Outreach and Recovery Team (SORT).
You will workclosely with other services to provide specialist mental health assessment andevidenced based treatments for people with severe and enduring mental healthdifficulties and complex presentations. You will be able to provide specialist clinicalinput and use a range of approaches to work with complexity, and to superviseand support junior team members of multi-disciplinary mental healthprofessionals.
The candidate willwork as an autonomous practitioner managing and prioritising individualcaseload and will act as a key worker within the personalised care framework.The post holder will be responsible for the assessment of care needs and thedevelopment, implementation and evaluation of programmes of care for a definedcaseload of people with complex mental health and/or social care needs.
We are seeking registered nurses who possessrecent clinical experience in a community or relevant mental health inpatientsetting.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Thepost-holder is responsible to the Manager of the Specialist outreach recoveryteam for the assessment of recovery needs, development, implementation andevaluation of care for patients with a primary diagnosis of complex psychosis.
Thepractitioner is responsible for assessment of recovery needs, development,implementation and evaluation of care, within agreed personalised careprocedures.
Thepost holder may be required to supervise junior staff within SORT.
Thepost holder will undertake assessments as part of the MDT routinely for all newreferrals as appropriate, at home or in a clinic setting, in accordance withthe agreed Operational Policy.
Thepost holder will provide nursing care and recovery interventions as appropriatein the right setting, in accordance with the agreed Operational Policy.
Thepost holder will provide community based recovery support for a defined groupof patients within the City. The post holder will work closely with all otherclinical professionals e.g. Consultant Psychiatrist, Clinical Psychologist,Social Workers, Occupational Therapist both within SORT as well as liaisingwith other Professionals externally where appropriate.
Thepost holder will act as a key worker for a defined group of practice patientsand organise care in accordance with the personalised care framework.
Thepost holder will be expected to attend and contribute to weekly MDT meetings,daily morning meetings and caseload formulation sessions within the team.
Thepost holder will support senior members of the team with day to day managementof the team and medication administration.
Thepost holder will be responsible for supporting the team with improving quality,and will be expected to attend training provided in house as well as mandatorytraining to maintain high standards of knowledge and skills.
Thepost holder will be expected to undertake a clinical lead for the team ineither an area of interest to the post holder, or an allocated lead dependingon the needs of the service.
Thepost holder will ensure that their clinical practice is based on evidence andgood practice and in accordance with any NICE guidance.
Thepost holder will be individually accountable for their practice in accordancewith the NMC professional code of conduct.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
