Job description

Location:Community role, base St Clare Hospice, Hastingwood, CM17 9JX, delivering care in the community across West Essex

Hours:37.5 hours per week(part time considered)

Shift pattern: Long Day 08:0020:00 Early 08:006:00 and Late 12:00-20:00

We are currently recruiting to the Hospice at Home team where patients and their families are at the heart of everything we do.

Main duties of the job

To work as part of a multi-disciplinary team to provide a high standard of specialistpalliative care to patients and their families, ensuring that high standards of care aremaintained.

About us

St Clare Hospice is a charity providing specialist palliative care to improve the lives ofpeople affected by a life-limiting illness in West Essex and the borders of EastHertfordshire. Our care helps patients, their families, and carers, to make the most ofevery moment together, no matter how long they have left to live. We support them both athome and in the Hospice.Our Hospice at Home team provide practical and psycho/social support for patients whowish to remain in their own home.

Job responsibilities

Clinical

To be a safe practitioner and work within the NMC Code of professional standardsof practice and behaviour.

Provide holistic assessment of patients and families care needs, plan interventionsrequired and ensure those interventions are actioned and their effectivenessevaluated.

To report directly to HH Nurse Team Lead.

To communicate effectively with patients and families, ensuring they are activelyinvolved with their own care planning.

To support patients and families through grief and loss and the life changingconsequences of a palliative diagnosis.

To liaise with appropriate members of the multi-professional team internal andexternal to the organisation in order to ensure co-ordination and continuity of careon discharge and a smooth transition between different care settings.

To maintain clear, accurate and legible patient documentation.

To work across 08.00 20.00 service hours, including weekends, and bank holidaysas required.

Education

To supervise and support junior colleagues, volunteers and students workingwithin the Hospice

To actively participate in Multi-Disciplinary Team Meetings and ensure thataccurate information is fed back to the appropriate members of the multidisciplinary teams.

To take responsibility for own ongoing education and professional developmentplan.

To support and participate in the education and development of colleagues.

To be involved in the training programme of professional visitors learners whereappropriate.

To mentor new staff members and students.

Management

To act as a role model for all colleagues, providing encouragement and motivationto achieve goals for the service.

To take charge of coordination for HH caseload, maintaining high standards ofcare escalating issues as required.

To take responsibility for the care plan of Hospice at Home patients during shift.

To Risk assess and maintain a safe environment for Hospice at Home staff whileon shift in community.

To be aware of cost implications and make effective use of resources.

To apply evidence based practice in their own nursing care and to encouragecolleagues to do the same.

To work within other clinical areas within the Hospice as required.

To actively participate in multidisciplinary team meetings and ensure that accurateinformation is shared with community team.

Quality

To actively participate in, quality improvement, clinical audit, and research as ameans of monitoring and improving standards of care.

To take responsibility for the role as a link nurse within the Hospice at Home Teame.g. tissue viability, infection control etc.
