Community Cardiac Nurse - 37.5 hour per week fixed term contract until 31/03/2026.

An exciting opportunity has arisen for a confident, dynamic and proactive nurse to join our service in Southampton.
Based at Bitterne Health Centre, we are a busy service with a diverse caseload, working in venues across the city and beyond. You will be involved in the delivery and running of community cardiac rehab classes and will strive to support patients and their families by providing education regarding their diagnosis and symptom management, secondary prevention, and risk factor management.
The successful applicant will have a passion for working in the community, great communication and interpersonal skills. You will need to be a good team player and be able to develop collaborative working relationships with your colleagues.
A certain amount of travel will be involved in this role as many of our clinics are held outside of Southampton in community venues, so flexibility is needed.
The service is predominantly Monday – Friday excluding bank holidays; however, some occasional Saturday work may be required.
A full driving licence and access to a car is essential for this community post.

The purpose of this role is to support the Community Cardiac Service in providing clinical, team-orientated, specialist care for people in Southampton and parts of Hampshire with Cardiac Disease, Heart Failure and associated conditions.
This role is an opportunity for a Registered Nurse with an interest in Cardiology to gain experience and skills within the community setting, working directly with the Specialist Cardiac Nursing Team.
The focus of the Community Cardiac Service is to ensure that patients are identified, treated and supported in the least invasive setting possible. Reduction of hospital admissions and reduced length of stay are key targets for this team.
The service supports a cardiac rehabilitation programme for Southampton City and Hampshire patients with qualifying conditions (stent, myocardial infarction, surgery, valve replacement/repair and transplant). These sessions take place across Hampshire in four sites: Totton, New Forest, Shirley, and Hedge End.
