Job summary

Kent Community Health NHS Foundation Trust is launching a new WorkWell Coach role to help people with health conditions move towards employment. You will deliver tailored coaching across multiple channels and work within a multidisciplinary team to support sustainable journeys to work.

As part of a pioneering service, you will assess needs, create action plans and provide ongoing coaching while liaising with partner services and employers. Some evening or weekend work may be required.

Qualifications

• Level 5 foundation degree or equivalent experience.
• Functional Skills at Level 2 in English and Maths.
• Recent experience supporting neurodivergent individuals and a full UK driving licence.

Responsibilities

• Provide tailored one-to-one support to a caseload, delivering coaching face-to-face, online and by telephone.
• Carry out holistic assessments, co-produce practical plans and connect participants with services.
• Build relationships with employers and partner organisations to support return‑to‑work planning.
