A mental health charity in Bradford is seeking a Wellbeing Practitioner to support adults facing severe mental health challenges. The ideal candidate will provide one-to-one support, design care plans, and work collaboratively with other services to ensure successful reintegration into the community. Strong communication and organisational skills are essential, along with a Level 3 qualification in a relevant field or equivalent experience. Hours : 34 hours per week

Probationary Period : 6 months

Bradford District and Craven Mind is a thriving independent mental health charity dedicated to promoting positive mental wellbeing and empowering people experiencing mental health challenges to manage and work towards recovery.
Our Personal Health Budgets service helps individuals transition from hospital back into the community by facilitating access to goods and services that promote recovery independence and overall well-being. Our SMI-LE service provides tailored support to people in the community experiencing mental health challenges, including assistance with mental health and emotional well‑being, loneliness and social isolation, social housing, welfare benefits, debt management and employment.
As a Wellbeing Practitioner you will provide one‑to‑one support to adults facing moderate to severe mental health challenges, addressing issues such as social isolation, housing benefits, employment and financial difficulties. You will design person‑centred plans of care and support people to settle back into the community following a hospital stay, working closely with secondary care colleagues and based in Community Mental Health Team sites.
