Job summary

A mental health charity in Bradford is seeking a Wellbeing Practitioner to support adults facing severe mental health challenges. The ideal candidate will provide one-to-one support, design care plans, and work collaboratively with other services to ensure successful reintegration into the community. Strong communication and organisational skills are essential, along with a Level 3 qualification in a relevant field or equivalent experience. Role Background

Our Personal Health Budgets service helps individuals transition from hospital back into the community by facilitating access to goods and services that promote recovery independence and overall well-being. Our SMI-LE service provides tailored support to people in the community experiencing mental health challenges, including assistance with mental health and emotional well‑being, loneliness and social isolation, social housing, welfare benefits, debt management and employment.

Role Purpose

As a Wellbeing Practitioner you will provide one‑to‑one support to adults facing moderate to severe mental health challenges, addressing issues such as social isolation, housing benefits, employment and financial difficulties. You will design person‑centred plans of care and support people to settle back into the community following a hospital stay, working closely with secondary care colleagues and based in Community Mental Health Team sites.

Role Outcomes
  • Provide face‑to‑face and telephone support for adults with moderate to severe mental health challenges.
  • Deliver support around client well‑being, including employment, welfare, benefits, social housing, money management, social isolation and bereavement.
  • Understand local support services and provide sign‑posting and onward referrals where appropriate.
  • Collaborate with other services across Bradford District and Craven to achieve the best outcome for service members.
  • Record information accurately and maintain up‑to‑date electronic service records.
  • Safeguard information governance and equality & diversity requirements and strive to improve system and practice.
  • Work in alignment with Bradford District and Craven Mind values and policies.
  • Perform any other duties reasonably required to support smooth operation of Mind in Bradford.

Qualifications
Essential
  • Level 3 or above qualification in a relevant field or at least 2 years of relevant experience.
Experience

  • Understanding of key issues within mental health and potential challenges faced by people.
  • Knowledge of other health & social care organisations across Bradford District & Craven.
  • Knowledge of risk management and health and safety issues.
  • Understanding of safeguarding and good practice.
  • Understanding of confidentiality and data protection.

  • Experience of barriers people face following discharge from a health setting.
  • Experience of housing application and barriers for re‑housing benefit application and processes.
