Solent Mind is seeking a Wellbeing Assessor to support patients experiencing mental health challenges in the Hampshire region. You will conduct assessments to understand needs and identify suitable support within the Primary Care Mental Health Service framework.

Working within the Avon Valley Medical Practice Network, you will join a multidisciplinary team including GPs and wellbeing assessors to provide compassionate, joined-up care across the New Forest coast.

Responsabilidades

• Undertake assessments of patients to understand their individual needs and identify the most appropriate support.
• Work collaboratively with the Primary Care Mental Health Service and associated organisations to provide targeted care.
• Help patients connect with community services, refer to specialist support, or access ongoing wellbeing opportunities.
