We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Visiting Clinical Care Practitioner /Community Matron. We have already seen fantastic results from our current Community Matron, who works primarily in local care homes. Because of this success, we are expanding the service to include our housebound patients, giving them better continuity, more personalised support, and high‑quality care in the comfort of their own homes. This is a wonderful opportunity to help shape and grow a service that truly puts patients first.

Main duties of the job

To deliver a high standard of patient care as a Clinical Care Practitioner (or Advanced Care Practitioner) in general practice, using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base. The postholder will:

  • Provide care for patients within the limits of their professional boundary, from initial history taking through to diagnosis, treatment and evaluation.
  • Deliver care in patients’ homes and at the surgery.
  • Identify, investigate and manage undifferentiated problems, including prescribing where the postholder is an independent prescriber.
  • Implement health promotion, quality assurance and clinical governance activities.
  • Maintain accurate records in line with GDPR and the Caldicott Principles.
  • Build effective working relationships with other health‑care professionals and with patients and their carers.
  • Support the delivery of the practice’s day‑to‑day clinical and operational activities, including health and safety and infection control.
  • Assist with the delivery of QOF targets and other finance‑related arrangements.
Qualifications

Essential:

  • Independent Nurse Prescriber
  • Mentor/teaching qualification
  • Recognised NP qualification at minimum degree level or equivalent
  • Clinical supervision training and experience

Desirable:

  • MSc Advanced Practice
  • MSc Level 7 modules pertinent to Advanced Practice
Knowledge & Skills

Essential:

  • Management of patients with acute, long‑term and complex conditions
  • Clinical examination skills (all systems)
  • Accountability for own role and local/national health policy
  • Clinical leadership, communication (written & verbal), negotiation and conflict management skills
  • Change management, teaching and mentorship in a clinical setting
  • Resource management

Desirable:

  • Knowledge of public health issues and determinants of health in the area
  • Clinical governance issues in primary care
Other Essential Qualities
  • Full UK driving licence
  • Self‑directed practitioner
  • Highly motivated, flexible, enthusiastic, team‑player, able to work across boundaries
Experience

Essential:

  • Recent primary and community nursing experience
  • Minimum 5 years post‑registration experience
  • Leadership in quality initiatives such as clinical benchmarking
  • Extended and supplementary nurse prescribing

Desirable:

  • Recent primary and community nursing experience
  • Patient‑led triage including telephone triage
  • Experience with community development initiatives
  • Health‑needs assessment of patients
