We are excited to offer an opportunity to join the dynamic and forward-thinking Frailty Virtual Ward team in Newham University Hospital. This innovative service is at the forefront of delivering integrated, patient‑centred care to some of our most vulnerable residents, supporting older adults with frailty to receive safe and effective treatment in the comfort of their own homes.
As part of the Frailty Virtual Ward, you will be working within a multidisciplinary team that includes GPs, geriatricians, community nurses, therapists, pharmacists, and social care colleagues. Together, we deliver proactive and responsive care to prevent avoidable hospital admissions, support early discharges, and improve quality of life for our patients.
