MAIN PURPOSE
Our Stop Smoking Practitioners deliver oneto one and group stop smoking interventions virtual clinics.
The post holder will be required to workflexibly across as required ensuring their skills, expertise and knowledge arefully utilised.
Thispost will:
Planand deliver evidence-based stop smoking advice and support.
Developand improve accessibility of the Everyone Health Stop Smoking Services throughthe pro-active promotion of the smoking cessation programmes and referralpathways amongst primary and secondary care services, other key health careproviders, allied professionals, community development initiatives, localagencies and the general public.
Provideongoing support to a caseload of referrals, supporting each quit attempt inline with national and company standards.
Contributeto the planning, delivery and development of the stop smoking service includingspecific project work.
SUMMARY OF RESPONSIBILITIES AND DUTIES
Inconjunction with the delivery team lead plan and deliver an appropriate andaccessible cessation service within the delivery team area.
Providecessation support including one to one and group support, assessing clientssuitability for Stop Smoking treatments.
Adhereto service protocols and procedures in relation to the provision of supportivepharmacotherapy medication.
Collateaccurate clinical records in line with national PHE/NCSCT requirements andinputting data into the service web-based database in a timely and professionalmanner.
Tosupport the service in delivering key performance indicators and activity asset out within the service specification.
Supportthe delivery of promotion activity across the services in line with the servicemarketing strategy.
Routinemaintenance and the checking and calibration of carbon monoxide monitors inline with service schedules to ensure validity of quits in line with departmentof health guidelines.
Workflexibly to provide a coordinated service across the localities to meet the needs of the client group. This will include work in the evening and some workon weekends where required.
Maintainup to date clinical knowledge, undertake evidence-based practice and promotethe use of new and innovative practice.
Inconjunction with delivery team lead, conduct analysis and evaluation ofdesignated programmes to improve the quality of service.
Beresponsible for identifying issues or concerns to line manager, so thatappropriate action can be taken.
Contributeto ongoing needs assessment, audit and other governance programmes in relationto activities as part of the health improvement agenda.
Tocontribute to the development, delivery and evaluation of smoking cessationtraining.
To takeresponsibility for own professional development in accordance with the outcomesfrom the annual development review, in order to meet the requirements of theKnowledge and Skills Framework and appraisal system.
Responsiblefor accurate uploading of own client data onto database whilst ensuring clientconfidentiality.
Ensurefollow up of clients is done in line with service protocol.
Toutilise a range of complex motivational, influencing and negotiating skills toincrease clients motivation and adherence to stop smoking.
Acommitment to continuous service improvement with expert insight into patientsneeds, so that all patients receive high quality care.
Understandingof harmful effects of smoking, smoking as an addiction behaviour, withdrawaland relapse
Clearunderstanding of the range and appropriate use of different smoking cessationinterventions
