Job summary

Torbay and South Devon NHS Foundation Trust in Paignton is seeking an experienced Urology Nurse Practitioner (NHS AfC: Band 6) to deliver specialist advisory and autonomous clinical care in the urology outpatient setting.

You will assess, plan, implement and evaluate nursing care, coordinate with colleagues, support patients and families, and contribute to nurse-led clinics including intravesical chemotherapy, following COSHH and local guidelines.

Urology Nurse Practitioner

Job overview

The Urology Nurse Practitioner's main aim is to provide specialist advice and clinical practice to patients within the urological outpatient setting and enabling optimum quality of life.

You will work as an expert practitioner assessing and initiating treatment as appropriate for patients within agreed protocols and work autonomously as a specialist resource for urology patients, their carers and other professionals.

You will also provide information, psycho-social support and advice to patients, carers and families undergoing treatment and investigations for urological procedures.

Main duties of the job

• Work as an expert practitioner by assessing, planning, implementing and evaluating the specialist nursing care of patients requiring urological investigations and treatments.
• Provide expert advice to clinical staff on urological issues.
• To act as the patients, advocate when required and provide expert advice and comprehensive information, both written and verbal, on treatment options to enable patients to make an informed choice.
• Establish and maintain coordination of care for patients throughout diagnosis, planning, treatment and follow-up, liaising with all other service providers to ensure a smooth pathway.
• The provision of nurse led clinics for assessment and cancer follow-up, initiating investigations and procedures, making recommendations for treatment and referring to other agencies or discharging as appropriate.
• Work autonomously in providing ongoing support for a large number of cancer patients who have open access to the urology nurse practitioner service, including patients awaiting results, those newly diagnosed with urological cancer and others with advanced disease.
• To manage a caseload of patients requiring intravesical chemotherapy, including administration, assessment and follow up in accordance with COSSH and Local Guidelines.
• To offer support, counselling, and follow-up to patients awaiting results and those newly diagnosed with urological cancer including communicating highly sensitive information to patients and their carers.

The Urology Outpatient service sees both cancer and benign patients in the outpatient setting, running nurse and consultant led procedure and investigation clinics daily. We offer support and advice to patients and other professionals within the acute and community setting and work alongside other departments to enhance the patient’s pathway of care.

The Urology team is a small, friendly team that is currently across multiple sites with the main outpatient clinics based in Paignton. Detailed job description and main responsibilities

Communication and Working Relationships

• The Nurse Specialist for the Urology Team will maintain excellent working relationships with patients, their relatives, and a multi-disciplinary team of professionals working across the trust, including:
• Modern Matron and Directorate Management / Executive Team
• Allied Health Professionals – Specialist Nurses, Ward Nurses, Consultants, Doctors, Diagnostics, Council services etc
• Outpatient services
• Administrative services
• Other urology nurse specialists within the south west STP
• Provide, receive, interpret and communicate highly specialist information with a range of health care professionals
• Present complex, sensitive or contentious information to large audiences.
• Establish excellent working relationships with patients communicating in a number of ways including virtual consultations, face to face, written, telephone etc.
• Communicate sensitive / distressing information and manage patient expectations
• Able to communicate effectively in demanding and challenging situations
• Ensure effective and prompt reporting of nursing issues and concerns to the matron or associate director of nursing and professional practice
• Actively participate in multi-professional meetings, acting as a patient advocate and represent nursing views
• Provide specialist knowledge to other areas, when assistance/advice is required

Planning and Organisation

• Assist the development of services for patients presenting with urology related health concerns
• Effectively assist with change in order to meet national / patients / service needs
• Co-ordinate multi-disciplinary team working in the specialist care of patients with urology related health concerns
• Maintain a caseload of patients and demonstrate advanced practice in assessment, planning, implementation and evaluation of care
• Collect, collate, evaluate and report information
• Maintain accurate patient records
• Ensure that national, regional and local guidelines are implemented, whilst continually monitoring the quality of care provision
• Be flexible and work within the rostering KPI guidelines
• Participate in clinical research, audit and contribute to the development of knowledge / education in urological related health concerns
• Effectively work within the service to ensure that it is responsive and flexible
• Manage own time with conflicting priorities to ensure that the needs of patients, workforce and service are met

Responsibility and Accountability

• Be a source of expertise in the care and provision of services for patients with urology related health concerns, liaising with the Lead Clinical Nurse Specialist for advice and support
• Provide specialist assessment of patients presenting with urological related health concerns and complex needs. Develop individualised patient care plans
• Deliver nursing care that is contemporaneous, evidence based and is reflective of best practice
• To be accountable for the quality of patient care provided within the service.
• Maintain own personal and professional development, undertake all mandatory training required for trust compliance and for the development of the role
• Develop, maintain and provide educational resources to all healthcare professionals in the trust who are working with patients who have urological related health concerns
• Promote patients’ equality, diversity and rights
• Participate in clinical research and audits. Implement change to the service based upon interpretations of the research findings
• Provide a positive learning environment to meet the needs of nursing and allied health professional students

Policy and Service Responsibility

• Support the development of patient pathways, policies and protocols in relation to the care of patients with urology related health concerns
• Work collaboratively with the lead nurse and clinician in the development and implementations of clinical pathways and protocols
• Assist the development and implementation of strategies to meet the educational needs of patients and carers so that they may be empowered to manage their condition and seek appropriate help and advice
• Assist the Development of new pathways, protocols and education in response to emerging knowledge and best practice
• Promote patient and public feedback through local initiatives to drive innovation and service improvements
• Participate in the implementation of clinical policy
• Establish robust audit criteria for the ongoing collection of information
• To have knowledge of quality assurance systems and clinical governance

Responsibility for Finance, Equipment and Other Resources

• Ensure flexible working and working to rostering KPI guidelines.
• Be aware of and assist budget holder for efficient and effective usage of the available resources, working within budget
• Ensure the effective and efficient use of physical & financial resources
• Develop and provide educational materials as appropriate for patients/family regarding their disease, treatments and services

Responsibility for Supervision, Leadership and Management

• To assistin the provision of expert advice and support for all allied health professionals, patients and carers
• Work with the multi-disciplinary team to continually appraise and review the service provided
• Act as a role model demonstrating high standards of care
• Demonstrate, monitor and maintain competency in clinical skills
• Contribute to the pre-registration nurses’ educational experience, acting as a mentor when supporting students
• Work within codes of practice, professional guidelines, NICE guidance and Patient Group Directions
• Actively participating in the educational programme of all staff, including students. Close liaison with the Practice Development Department, offering practical and clinical guidance and attending lectures and meetings as required
• Ensure clinical governance is maintained through incident reporting and reviewing

Information Technology and Administrative Duties

• Maintain an accurate record of patient caseloads and contact visits. Take part in the development of IT systems pertaining to patient contact in line with the data requirements of NHSEI and PHE
• Ensure familiarity with relevant computerised systems integral to the running of the service
• Provision of information sharing where appropriate, on a need to know basis
• To carry out regular audits as required and maintain data collection regarding patient interactions and service provision
