About the Role

Following a promotion within the service, an opportunity has arisen for Band 7 Nurse/Paramedic within the Urgent Community Response Team (UCR). In this role, you will deliver a rapid 2‑hour response to patients experiencing a health crisis, aiming to prevent unnecessary hospital admissions and support care in the community as part of the Virtual Ward core clinical delivery team.

Clinical Responsibilities
  • Deliver short‑term, intensive case management to patients in crisis who are at high risk of unplanned hospital admission, typically for up to 72 hours.
  • Respond promptly to patients requiring urgent assessment, working collaboratively with patients and their support networks to develop management plans that enable safe care at home.
  • Support ambulatory care pathways for conditions such as UTIs, pneumonia and cellulitis, including the initiation and administration of IV therapies.
  • Work closely with Virtual Ward colleagues and other members of the community integrated team to ensure seamless care and continuity for patients.
  • Play a key role in preventing unnecessary hospital admissions by delivering timely, patient‑centred interventions and coordinating care with wider community services.
  • Act as a senior clinician within the UCR team, offering guidance, mentorship, and support to junior staff and contributing to a culture of excellence and continuous learning.
  • Undertake line‑management duties including supervision, appraisal and professional development of team members.
  • Lead and contribute to quality improvement initiatives, using data and feedback to inform service development and enhance patient outcomes.
  • Play an active role in the ongoing integration of UCR with other services, including the Virtual Ward and Integrated Night Service, helping to shape a responsive and cohesive 24‑hour care model.
  • Promote evidence‑based practice and contribute to the development of clinical guidelines, pathways and protocols that support safe, effective and person‑centred care.
