Unscheduled Care Practitioner

This is an exciting opportunity for a qualified nurse/paramedic practitioner to join a team of urgent care practitioners in North Lincolnshire. The successful candidate will be responsible for delivering urgent care services across the community to provide an integrated service model that reduces waiting times, unnecessary A&E attendance, hospital admission and ambulance conveyances.

Responsibilities
  • Work as an autonomous practitioner, providing clinical care including telephone triage, diagnosis, treatment, referral, review or discharge.
  • Utilise a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions.
  • Deliver services that support a "No Wrong Door" approach at the time of crisis.
  • Contribute to the delivery of the two UCR services in North Lincolnshire and may engage with virtual ward opportunities.

Education and qualifications
  • Level 1 registered nurse or state registered paramedic.
  • Degree or master’s in nursing studies or a relevant paramedic qualification.
  • Specialist Practitioner qualification or equivalent.
