A community health service in the Shrewsbury area seeks a Registered Nurse to provide high-quality care to patients at home, ensuring safe alternatives to hospital admissions. The role requires a valid Nurse Registration, clinical judgment, and the ability to collaborate with a multidisciplinary team. The position involves flexible working hours across a 7-day service. Continuous training and professional development are offered.
An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service. The post will form part of a seven day service based within the Shrewsbury locality team. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.
We are looking for a Registered Nurse who shares our passion for providing excellent quality of care and can help devise & deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.

You will also need to be flexible to work rotas between 08:00-00:00, 7 days a week including bank holidays on a rota basis, to meet the service needs. You may on occasion be required to work in alternative community locations to meet the service requirements.
As part of your role and to further develop your clinical skills, the service is also committed to providing clinical support, continuous professional development & training.
