Triage Nurse/Paramedic – UCRT SPA

Triage Nurse/Paramedic Band 6 | Hours – 37.5 hours per week | Location – Rotational Post – Rochford/Basildon

An exciting opportunity has become available for a Band 6 Nurses/Paramedics to join our Urgent Community Response Team (UCRT) Access and Clinical Triage Team.

UCRT is a partnership of community admission avoidance teams across Mid and South Essex, operating in collaboration with EPUT, NELFT and Provide. The UCRT Access and Clinical Triage team is responsible for the triaging and acceptance of referrals and is currently hosted by EPUT.

UCRT services provide specialist care in patients’ own homes, making contact within two hours of receiving a referral to stabilise their immediate health and social needs and plan ongoing care and intervention as appropriate.

Job Responsibilities
  • Support the triage function of the UCRT access team by undertaking telephone assessments of referrals from the Ambulance Service, Care and other health and social care professionals, determining suitability and safety of patients for assessment by local UCRT services or coordinating a response by another appropriate community‑based team or service.
  • Cover a rota to undertake triage within the UCRT Single Point of Access and, where required, in an integrated/in‑reach function within 111 and 999 control centres to 'pull' referrals deemed as safe and appropriate for a UCRT response (Hear & Treat).
  • Carry out the clinical triage of referrals received in the UCRT Single Point of Access.
Qualifications
  • Registered Nurse or Paramedic.
  • Completion of a Triage/Assessment Course.
Experience
  • Relevant experience to UCRT.
  • Experience of undertaking triage.
  • Experience of using alternative community pathways.
