Job summary

The UCCH is an award winning innovative multi disciplinary hub aimed at facilitating improved patient outcomes, admission avoidance and enhanced patient journeys. We are expanding our team in the UCCH with some additional part‑time Triage Clinicians to cover 7 hours per day across all 7 days in the week. Exact working pattern can be discussed at interview.

The position of UCCH Triage Clinician requires the post holder to practice autonomously to triage and risk assess category 3-5 cases in the EEAST and refer on to the most appropriate service in line with NHS England Community Health Services two‑hour urgent community response standard guidance. By working alongside other providers in primary care, secondary care, mental health, palliative care, community and voluntary organisations, the UCCH aims to manage patients within the community, keeping care closer to home.

Main duties of the job
  • Work within the UCCH Multi‑Discipline Team to review low acuity cases held in the East of England Ambulance Service (queue and identify cases that may be suitable for coordinated community‑based care)
  • Autonomously triage cases in line with NHS England Community Health Services two‑hour urgent community response standard guidance
  • Pass cases via Pathways Clinical Consultation System PaCCS and the Directory of Services to the most appropriate provider for further management
  • Complete remote triage via telephone or video
  • Keep accurate and contemporaneous clinical records
  • Safeguard patients and make safeguarding referrals where necessary
  • Provide quality, evidence‑based health care, in line with protocols and policies to meet the immediate needs of the patient and provide support to other team members working in the UCCH
About us
Essential
  • Qualified as a RGN / Paramedic / Allied Healthcare professional – holding full registration and professional indemnity
  • Ability to triage in a remote setting, across patients of all ages
  • Previous experience working within NHS 111, CAS or Community Care
  • Knowledge of how other system partners and health and social care services operate (e.g. primary care, urgent treatment centres, emergency department, pharmacies, ambulance service)
Desirable
  • Completed PaCCS training
  • Experience with video consultation

Interviews will take place between 13th – 25th November.
