You will be based at Derby City Council House, with the potential requirement to work across the wider Derby City region. You will be expected to provide a high standard of person-centred clinical care to housebound patients, many of whom may be frail, elderly, and have complex healthcare needs.
You will receive support from a community GP and other experienced, qualified members of the multidisciplinary team, providing an excellent environment for developing your advanced clinical practice skills.
Due to the nature of the role, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business purposes.
If you have a passion for life-long learning, enthusiasm for supporting the development needs of others, excellent communication skills and a commitment to continuous quality improvement we would love to hear from you!

As an ACP you will:
You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
It is an exciting time to join primary care and community services as the national Ageing Well programme gathers pace.
DCHS is working closely with the Primary Care Networks to transform the care of housebound and vulnerable people across Derbyshire through our Team-Up approach.
