Job description
Job summary

The post holder will be responsible for delivering basic practice nursing services within the practice. They will work with support from senior practice nurses and the clinical nurse manager to deliver high‑quality care, and will work within all aspects of practice nursing after completion of training and competencies. The role involves supporting patients to be healthy, monitoring long‑term conditions, providing health screening and ensuring patients’ health needs are met under local and practice policies. The post holder will also provide support to junior nurses, healthcare assistants, phlebotomists and student nurses as required.

Main duties of the job
  • Assess, plan, develop, implement and evaluate programmes to promote health and well‑being of patients attending the practice.
  • Participate in opportunistic and routine cardiovascular risk assessments, give health advice and make referrals to exercise, alcohol, smoking, weight management and mental health services where applicable.
  • Work with patients to support compliance with and adherence to prescribed treatments.
  • Support patients to adopt health‑promotion strategies that promote healthy lifestyles and apply principles of self‑care.
  • Support and manage the health needs of women presenting for family planning, cervical screening and sexual health consultations (cervical screening after completion of course).
  • Assess, identify and refer patients presenting with mental health needs; participate in annual mental health reviews.
  • Undertake ECGs as per practice policies, and perform venepuncture.
  • Administer medications or injections as per prescription, patient group directions or patient‑specific directions.
  • Assess, plan, implement and evaluate treatment for patients presenting with acute and chronic wounds, including removal of clips and sutures.
  • Collect, store and dispatch pathological specimens to a high standard (blood samples, urine, stool, sputum, swabs, smears, etc.), ensuring universal precautions are followed and manage the results, referring to GP as necessary.
