About the Team

We’re proud of our recently transformed Older Adult Community Mental Health Team, developed in line with the Community Mental Health Framework. This specialist pathway supports older adults with functional mental health needs and neurodegenerative disorders, where the impact on wellbeing and independence is significant.

Our team is dynamic, multidisciplinary, and deeply collaborative. You’ll work alongside Senior Practitioners from both the Local Authority and the Mental Health NHS Trust, united in our goal to achieve the best outcomes for people in our community.
