Overview

This is a Full Time, Permanent role with Bristol City Council. The post requires a social work qualification and registration with Social Work England to be shortlisted. An exciting opportunity has arisen for an experienced leader to head our 16+ children in care transitions team, working with care experienced young people as they transition to becoming care leavers.

Young people in our service have often entered care as older children or may present with more complex needs. The team manager will ensure comprehensive assessment of need, pathway planning, and oversee a multi‑agency plan to support the transition as the young person becomes a care leaver. Our work can be reactive and fast paced, and you will need to be trauma informed, able to work independently, and possess the skills to challenge constructively. Most young people live in and around Bristol, with some living further afield.

We recognise the importance of the whole network in building security and safety for our young people, ensuring they understand their life story and journey through care. Our practice is underpinned by signs of safety and systemic practice, and we encourage workers to be reflective in their approach. Responsibilities
  • Lead and inspire others; work systemically to develop the service for young people.
  • Oversee and line manage approximately seven to ten staff members and support 100 care‑experienced young people.
  • Ensure comprehensive assessment of need and effective pathway planning for each young person.
  • Maintain a multi‑agency approach to support transitions for care leavers aged 16+.
  • Be organised, flexible, and solution‑focussed in a fast‑paced environment.
  • Promote trauma‑informed practice and constructive challenge when needed.
Qualifications
  • Social work qualification and registration with Social Work England required to be shortlisted.
  • Enhanced DBS check is required for the role.
