This is a key role within the organisation; the post holder will lead and manage the Adult Social Care practitioners within the Haverhill Integrated Neighbourhood Team (INT). This will ensure that the highest standards of health, social care and reablement continue to be delivered within the locality.
In addition, the role supports the Locality Lead in achieving successful integration between health and social care by working collaboratively with stakeholders and partners across the system, with the aim of delivering improved outcomes for people in Suffolk. Building strong working relationships with all key partners is essential, ensuring that health and social care services are well-connected and accessible to those who need them.

The Haverhill Integrated Neighbourhood Team (INT) is a busy, supportive team that covers Haverhill and surrounding villages. We work closely with colleagues from community health, the voluntary and community sector, housing, NSFT and the wider multidisciplinary team to support adults of all ages remain as independent for as long as possible whilst ensuring their safety and wellbeing. We are committed to ensuring that the adults we support are the centre of all we do this requires us to be able to think creatively to ensure adults receive the best possible support, that works for them.
