Responsibilities

  • Lead and coordinate the care team on shift; allocate tasks and manage breaks.
  • Deliver high-quality, person-centred care and support with daily living.
  • Administer and record medications safely, escalating concerns promptly.
  • Complete and update care plans and daily notes accurately.
  • Carry out observations and report changes to nurses/management.
  • Conduct effective handovers and maintain clear communication.
  • Mentor, supervise and coach new starters and Care Assistants.
  • Liaise with residents' families, GPs and external professionals.
  • Uphold safeguarding, dignity, infection control, and health & safety standards.
  • Assist with audits, incident reporting and falls prevention.

  • Deliver high-quality, person-centred care and support with daily living.
  • Administer and record medications safely (eMAR), escalating concerns promptly.
  • Complete and update care plans, risk assessments and daily notes accurately.
  • Carry out observations (e.g., BP, pulse, weight) and report changes to nurses/management.
  • Conduct effective handovers and maintain clear communication across the team.
  • Mentor, supervise and coach new starters and Care Assistants; support training compliance.
  • Liaise professionally with residents' families, GPs and external professionals.
  • Uphold safeguarding, dignity, infection control, GDPR and health & safety standards.
  • Assist with audits (care records/meds), incident reporting and falls prevention.
