Job summary

Creative Support is seeking an experienced Team Leader to coordinate learning disabilities supported housing schemes and domiciliary services across Blackpool. You will deploy staff to meet individual needs, identify mandatory and service-specific training, and undertake initial assessments to develop co-produced support plans with designated Key Workers.

You will drive person-centred reviews and maintain a safe, welcoming environment.

Responsibilities

• Oversee deployment of staff to meet individual needs within available resources
• Identify mandatory and service-specific training needs
• Undertake initial assessments of need and ensure co-produced support plans with designated Key Workers
• Establish clear goals and pathways towards greater independence for people we support
• Organise regular person-centred reviews incorporating users' views into planning and decision-making
• Provide holistic wrap-around support and work with multi-disciplinary teams and community agencies
• Maintain high standards of environmental safety and quality and foster a welcoming atmosphere
• Monitor quality using KPIs and plan actions to improve services
• Demonstrate strong safeguarding duties and risk management to protect vulnerable individuals
