Main duties of the job

Exciting Opportunity! Join the Rapid Response Team as a Community-Based Team Lead

As a Team Lead, you'll manage staff, oversee rotas, recruitment, and supervise learners. You'll monitor sickness absence, annual leave, and training, ensuring compliance with trust policies. You'll play a key role in delivering the Hospital@Home service, ensuring timely, appropriate care is provided in patients' own homes.

You’ll work closely with patients and carers to manage health needs, promote self-care, and develop personalized plans. Collaborating with multidisciplinary teams—including NWAS, GPs, pharmacy, and the wider Neighbourhood Team—you'll help shape high-quality, patient-centered services aligned with trust goals.

Person Specification

Qualifications

• Registered Nurse with current NMC/HCPC registration
• Relevant Professional Health related degree
• Recognised teaching/mentor qualification
• V300 (non-medical prescribing) or willingness to undertake
• APLS / ALS
• possess a recognised clinical skills module at masters level

Experience

• Extensive knowledge/clinical experience in Urgent and or Community setting
• Knowledge and understanding of local health and social care pathways
• Experience in the management of long term conditions e.g. dementia, COPD, CHD etc
• Understanding of frailty and end of life processes
• Previous experience or involvement with the writing of policies and protocols
• Experience of leading, planning and completing audit /research

Skills

• Promotes and supports own professional development
• Willingness to undertake further study as required for the post
• Ability to travel to different geographical locations across the Trust footprint in line with service needs
• demonstrates an understanding of HR policies and procedures
