Job description
Position Overview

We are looking for an experienced RNLD or RMN to manage the day-to-day running of the Primary Care Liaison Team based in the City and County of Nottingham and the Acute Liaison Team based in NUH Hospitals. The post holder’s key role is to lead the primary care team to support adults with intellectual disabilities to access mainstream health services. The post holder will lead on the development of GP registers for those identified as having a learning disability registered with a GP in Nottingham City and Nottinghamshire.

Role and Responsibilities
  • To provide effective leadership to all staff within the allocated team.
  • To demonstrate clinical leadership through personal practice.
  • To manage and monitor daily staffing levels and skill mix to comply with agreed staffing levels, providing an effective and safe service.
  • To be responsible for recruitment and selection, performance management, sickness absence, appraisal and personal development, grievance and disciplinary issues in accordance with HR policies.
  • To identify learning and development needs of staff and facilitate the required training.
  • To provide clinical advice on complex issues and referrals to members of the team.
  • To work collaboratively with the care team in facilitating timely assessments within the service.
  • To ensure care is delivered within evidence of best practice.
