A healthcare organization in the UK seeks a Patient Care Navigator to guide patients through their surgical journey from pre-operative planning to recovery. This role is essential in enhancing care coordination and patient engagement. Acts as a central point of contact in the surgical practice, guiding patients through their entire surgical journey from pre-operative planning through recovery. This role is crucial for improving care coordination, educating patients, and removing barriers to treatment. PCN’s are individuals with excellent communication and interpersonal skills who establish trusting professional relationships with patients. The PCN, under the general direction of the office manager, provides patient and practice support to enhance patient engagement in meeting care plan goals. The PCN works with the practice to close care gaps and works closely with the surgical teams to identify and remove barriers to care.
