Bath and North East Somerset Council is seeking an experienced care and support professional to join the Supported Living Service as a coordinator. The role supports people with learning disabilities, autism and complex needs, helping them live independently and maintain their tenancies.

The coordinator will manage rotas, supervise staff, contribute to care reviews and lead delivery of services across property and area-wide provisions, including every-other-weekend shifts.

Qualifications

  • Care experience in a CQC registered service or close equivalent.
  • Strong transferable skills for diverse environments.
  • Ability to work in a physically demanding role and meet care needs.

Responsibilities

  • Plan rotas in collaboration with deputy managers.
  • Supervise staff and contribute to care reviews.
  • Lead service delivery and oversee daily/shift/weekend plans.
  • Work every other weekend to ensure consistent support.
  • Seek advice from on-duty deputy/manager as required.
  • Update support plans and risk assessments.
  • Conduct competency checks and limited audits.
  • Induct new staff and participate in on-call rota.
  • Arrange onward referrals for specialist support.
  • Handle safeguarding referrals and attend meetings.

This is an exciting opportunity for an experienced care and support professional to join our Supported Living Service as a coordinator.

About the Team

We support a range of service users at various properties in Bath & North East Somerset. The majority of the people we support have learning disabilities, many also have autism and/or other complex needs. We support people to maintain their tenancies and to live as independently as possible. We are a CQC registered service.

Our coordinators work under the guidance of the registered manager and deputy managers and will be assigned key duties at specific properties and associated geographical areas. Coordinators are also expected to work flexibly across our area-wide provision to ensure consistent support is in place. Please see Role Specific Criteria for further information.

About the Role

We will consider applications for 22.5 hours per week up to 37 hours per week for this role.

Our coordinator tasks include:
  • Planning of rotas in collaboration with deputy managers
  • Supervision of staff
  • Contributing to care reviews
  • Leading the delivery of the service, modelling good support and overseeing the plan for a day, shift or weekend
  • Working every other weekend to ensure people are supported and any actions required are followed upSeeking advice from the on-duty or on-call deputy or manager as required
  • Support plan and risk assessment updates
  • Competency checks and limited audits
  • Induction of staff
  • Participation in on-call rota
  • Onward referrals for specialist support
  • Safeguarding referrals and participation in meetings

About You
  • Care experience – such as having been a support worker in a CQC registered service or close equivalent.
  • Transferable skills

  • A Level 3 diploma in Health & Social Care or a close equivalent qualification.
  • A passion for delivering quality, person centred support.
