Job summary

Royal Surrey NHS Foundation Trust is seeking a part-time Care Support Worker (Band 2) to join the Flexi-Bank on a 1-year basis. The role is based at Rosewood, Farmfield Drive, Charlwood RH6 0BN, with early, late and night shifts across a 37.5-hour week and bankshift arrangements through the Flexi-Bank service.

You will be part of a professional team supporting people with learning disabilities, autism and complex health needs, working with relatives and other pros to improve lives and promote

Qualifications

  • Driving Licence suitable for the United Kingdom.
  • Level 2 Qualification in health and social care related topic.
  • Experience providing care or support to an individual, paid or unpaid.
  • To drive Trust transport in line with service needs.

Responsibilities

  • Support people using our services in line with their individual care plan to promote as much independence as possible.
  • Assist with all aspects of personal care (continence, showering, bathing) per individual needs.
  • Monitor health and behaviour of people using the service.
  • Report and record relevant information and notify senior staff of changes.
  • Act as a key-worker for named individuals within the service.
  • Promote, support and deliver activities within the service and wider community.
  • Provide person centred support ensuring respect, privacy and dignity.
  • Maintain relationships with families and carers, and involve people in their care planning.

Surrey and Borders Partnership NHS Foundation Trust is looking for someone who will be a part of a professional team supporting people with learning disabilities, autism, complex health needs in the service they access and their wider community. You will be working closely with relatives, carers, friends, advocates, colleagues and other professionals.

This role is suitable for someone who has passion, drive and commitment to improve the lives of people with a learning disability.

Work Location: Rosewood, Farmfield Drive, Charlwood Road, Charlwood RH6 0BN

Working Shift Pattern: Early 07:00-14:30, Late 13:30-21:00, Night 20:45-07:15

Detailed job description and main responsibilities
  • Support people using our services in line with their individual care plan to promote as much independence as possible
  • To assist with all aspects of personal care (supporting continence needs, showering, bathing etc.) in accordance with individual’s support needs.
  • Participate in the ongoing monitoring of the health (mental and physical) and behaviour of people using the service.
  • Reporting and recording all relevant information and reporting to senior staff any changes in the person
  • Act as a key-worker for named individuals within the service.
  • To promote, support and deliver a range of activities within the service and in the wider community.
  • To provide person centred support as identified within the care plan ensuring respect, privacy and dignity at all times.
  • Help to maintain and develop relationships between individuals and their families, friends and significant others.
  • Participate in and contribute to meetings.
  • Involve people in the development of their own care and support packages.
  • Ensure confidentiality in all matters relating to each person is maintained.
  • To maintain the safety of people at all times, alerting senior staff to any identified risks whether perceived or actual.
  • There is an amount of manual handling involved including assisting people with walking, sitting and other activities of daily living and by means of wheelchairs and mechanical aids within recommended guidelines.
  • The post holder is asked to familiarise themselves with the Social Care Charter for the Division and implement the behaviours within it in their daily work dutie
Person specification
Qualifications
  • Driving License suitable for the United Kingdom
  • Level 2 Qualification in health and social care related topic
Experience
  • Providing care or support to an individual, either paid or unpaid
  • To drive Trust transport in line with service needs.
  • A driving licence suitable driving in the U.K, and meets the requirements set out by the Trust insurers to drive the Trust vehicle.
