The role involves medication assistance, personal care, and helping individuals engage in activities in a welcoming hub and local community.

We are seeking experienced social care professionals to join our Day Centre and Community Outreach services in Perth. If you are passionate about supporting adults with complex disabilities and want to make a real difference in their lives, we would love to hear from you.
Our Support Workers love working between the variety of settings our services have on offer. Assisting adults with complex disabilities to enjoy our purpose-built hub setting, which offers; sensory, kitchen and craft spaces with access to idyllic grounds and amazing facilities such as a hydrotherapy pool, rebound therapy an accessible gym. This includes support with medication, personal intimate care and mealtimes.
In addition to working in the day centre, you will also support our customers in the wider community through our Community Outreach services. You will enable people to live independently in their own homes and participate in community life, offering support with personal care, medication, and social activities.

Our day centre is based at the Upper Springland Campus, Isla Road, Perth, PH2 7HQ and Outreach services are in central Perth and surrounding areas in customers own homes & wider community.
