What you'll do:

You will provide outcome-focused, person-centered support that promotes independence and maximises opportunities for the people we support. Your role will include:

  • Supporting with medication management
  • Teaching and encouraging life skills
  • Assisting with daily living tasks
  • Facilitating community access and engagement
  • Providing personal care
  • Helping manage tenancies
  • Liaising with other healthcare professionals to ensure clients receive the best possible support
  • Supporting clients with personal care

We need full-time (37.5 hours) and part-time (23.5 hours) staff ready to work across day and night shifts, working around a 7-day rota and you will be required to be on call on a rota basis.
