AHP Support Workers are a vital part of our healthcare teams in the community.
This fixed-term/secondment role is based within the Exeter Community Rehabilitation team, which includes Physiotherapists, Occupational Therapists, a Rehabilitation Nurse, and an Assistant Practitioner.
The service supports patients with long-term and acute conditions, falls prevention, and care of the older person, aiming to help them reach their full potential. The ideal candidate will be highly motivated to provide patient-centered care and have experience in health and social care settings.
Support is provided by Clinical Leads, and a structured in-service training program is available for development.
The team collaborates closely with Community Nursing and social care teams, and supports the Community Urgent Response team when needed.
The South & West cluster covers Broadclyst, Pinhoe, Topsham, Exminster, Alphington, St Thomas, and Starcross.
Working Pattern: Hours per successful candidate will contribute to a 7-day service on a rota basis.
