Swanton Care & Community Ltd in Barnsley is seeking a dedicated Support Worker to join our team across full-time and part-time roles.

You will provide practical assistance and emotional support to empower people to live independently within their community, with duties including activity facilitation, personal care, and recording progress.

Whilst fully embracing the company ethos support workers provide practical assistance and emotional support to empower people, we support to achieve their goals and aspirations, respecting everyone’s individuality and enabling them to make appropriate choices to live the life they want to live.Support is required 24 hours a day, 7 days a week.

• To provide support to empower the people we support to achieve their goals and aspirations, respecting everyone’s individuality and enabling them to make appropriate choices to live the life they wish, in line with their individual support plans.
• Promote the health, well-being, physical, emotional, cultural and spiritual beliefs of the people we support and ensure they are met in an inclusive way.
• To facilitate social, educational or work activities such as visiting family and friends, supported employment, walks, shopping and other recreational activities of their choice.
• General housekeeping duties that can include shopping, domestic duties as well as elements of personal care.
• As part of the role, you will need to be able to complete a range of records and documents.

About The Service

Supporting people in their homes to live an independent life within their community.

Our supported living homes are in close proximity to each other, and are nestled in the community of Darfield. We offer person centred support to some very complex individuals. As part of schemes in Barnsley, we can also provide social support for people living in the community based on each person’s hobbies and interests.
