Job summary

Salutem Care and Education is recruiting a Support Worker in Nottingham to provide high‑quality, person‑centred support for adults with learning and physical disabilities. You will help individuals live more independently, participate in their communities and access education, employment and activities.

About You
Key Responsibilities

As a Support Worker, you will provide high‑quality, person‑centred support that empowers individuals to live independently, achieve their goals, and participate fully in their communities.

Key Duties Include
  • Delivering personalised support in line with care plans, risk assessments, and Positive Behaviour Support (PBS) plans.
  • Promoting independence, choice, dignity, equality, and inclusion.
  • Supporting individuals with daily living skills, education, employment, volunteering, and social activities.
  • Assisting people to maintain positive relationships and engage in community life.
  • Supporting physical and mental wellbeing, including attending healthcare appointments and promoting healthy lifestyles.
  • Providing personal care, mobility support, and assistance with meals and specialist equipment where required.
  • Administering and recording medication safely in accordance with policies and training.
  • Supporting individuals with complex needs using proactive, positive behaviour support approaches.
  • Safeguarding individuals by recognising and reporting concerns promptly and promoting personal safety.
  • Maintaining a safe, clean, and welcoming home environment, including household tasks, meal preparation, and health and safety responsibilities.
  • Keeping accurate and timely records, including daily notes, health records, incidents, and financial transactions.
  • Supporting individuals with budgeting and managing finances where appropriate.
  • Working collaboratively with colleagues, families, healthcare professionals, and external agencies.
  • Contributing to quality assurance, compliance, training, and continuous service improvement.
