A leading healthcare provider in Llandudno Junction seeks dedicated Support Workers to provide person-centred care to adults with mental health challenges. Cwlach road is 5 bed flats property. Individuals are between 39 years to 53 years of age and live in their own flats. They have been discharged from hospital to re‑integrate in the community.
Experience of working with Mental Health and Mild Challenging Behaviour. No personal care required.

This primarily involves support using a person‑centred approach. Our support workers measure outcomes specifically to an individual’s needs and evidence using our person‑centred outcome‑measuring tool.
Successful candidates will be required to complete an enhanced DBS disclosure prior to commencement of employment; the disclosure expense will be met by the employer.
