As a Support worker, you will commit to delivering care with our customers in their own homes, enabling them to live independently close to family and friends, supporting them with all aspects of daily living.

You must be a driver and have your own car.

Roles and Responsibilities
  • Assist Client with all areas of personal care and hygiene.
  • Assist Client with all aspects of mobilising and transfers as per her bespoke support plans.
  • Support with meal planning and food and drink preparations.
  • Encourage Client to be as independent as possible.
  • Support Client to organise health appointments and make practical arrangements for travel.
  • Follow all care and support and therapy plans.
  • Support Client to identify/plan activities that she may enjoy.
  • Facilitate and follow risk management plans and inform the Case Manager of any concerns or changes required.
  • Support Client to manage her emotions and problem‑solve when situations are stressful or unfamiliar, and help decide what action is needed to make her feel more positive and facilitate an approach with Client for completing these actions.
  • Communicate effectively and positively with members of Client’s Multidisciplinary team.
  • Complete all necessary recordings and daily notes.
  • Complete all required training and attend team meetings and supervisions when arranged.
  • Follow all policy and procedures and agreed team policies.
