Health Now is seeking Support Workers to join our team and provide exceptional care and support to adults with complex needs in the community. As a Support Worker with Health Now, you will play a crucial role in promoting independence, supporting care plan interventions and enhancing the quality of life for our clients.

Responsibilities
  • Provide one-on-one support to individuals with complex needs.
  • Assist clients with daily living activities, personal care, and medication management.
  • Foster a safe and supportive environment, promoting physical and emotional well-being.
  • Collaborate with a multidisciplinary team to ensure care plans are being adhered to.
  • Support clients in building life skills and participating in daily social/community activities.
  • Document client progress, communicate observations to the Clinical Lead or Registered Manager.
  • Provide emotional support, manage challenging behaviours and ensure clients' voices are heard.
Requirements / Qualifications
  • Compassionate and empathetic with a strong desire to support others.
  • Previous experience in a support worker role is advantageous, minimum 3 years.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • A commitment to promoting independence and enhancing the lives of individuals with complex needs.
  • Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
  • GCSE, BTEC, NVQ or equivalent.

To be considered for this position, you will demonstrate exposure to the following areas of expertise: mental health, complex health needs, physical health needs, hospital discharge to community rehabilitation, autism and learning disabilities, life-limiting conditions, working with adults aged 18-65 and 65 and above.

Additional Requirements
  • A valid full UK manual driving license is essential.
