A leading healthcare provider in Bournemouth is seeking a dedicated professional to join their community rehabilitation team. The role involves providing care for individuals with severe mental health issues, working towards recovery and social inclusion. Responsibilities include managing care plans and maintaining high standards of patient records. The community rehabilitation team will support people to reach an optimum level of function, focusing on activities of daily living, vocation and leisure. The post holder will work towards these goals and will be responsible for providing care in a coordinated, person‑centred manner.
