Job summary

Phoenix Futures in Belmarsh is seeking a compassionate Substance Use Practitioner to support individuals with substance use issues. You will provide harm reduction, help develop relapse prevention plans and care coordination, and advocate for service users while signposting to relevant agencies.

The Role

As a Substance Use Practitioner you will be providing harm reduction to those with substance use issues and supporting in their recovery through relapse prevention and care planning. You will also act as an advocate for these service users, signposting and referring to the necessary agencies in order to support them in their recovery holistically.
